Chondroblastoma of the Lumbar Vertebra - A Case Report -

Abstract
Chondroblastoma is a benign, cartilage-producing neoplasm that acounts for less than 1% of all bone tumors. Most patients are between 10 and 25 years of age at diagnosis. There is a male predominace. Greater than 75% involve long bones. Chondroblastoma of the vertebra is very rare. We report a case of chondroblastoma arising in the lumbar vertebra in 19-year-old man. The patient presented with low back pain for 6 months. Physical examination revealed no abnormal sensory deficit. Plain radiogrpahs showed osteolytic change of the fourth lumbar vertebra. Computed tomography revealed an osteolytic lesion of the fourth lumbar vertebra. Positron emission tomography-computed tomography revealed a hypermetabolic lesion in the fourth lumbar vertebra. Multiple small nodular lesions were found in both lung. CT-guided needle biopsy for the fourth lumbar vertebral lesion was performed. Microscopic examination showed round to polygonal cells having invaginated or grooved nuclei and eosinophilic cytoplasm. Focal chondroid matrix was noted. Osteoclast-type giant cells were scattered. The pathologic diagnosis on needle biopsy section was chondroblastoma. The corpectomy was performed. Gross examination showed multiple fragments of brownish tan to gray, soft tissue and bone, measuring 3.5¡¿3.0¡¿3.0 cm in aggregate. On microscopic examintion, round, oval and polygonal-shaped, mononuclear cells were proliferated. The cells had round to oval nuclei and eosinophilic cytoplasm, The nuclear invagination or grooves were noted. Some cells had distinct cytoplasmic borders. Mitotic figures were occasionally seen. Osteoclast-type giant cells were randomly scattered. There were foci of eosinophilic chondroid matrix. Tumor cells invaded the cortical bone and adjacent soft tissue. Vascular invasion was present.
